Certificate(s) of Professional Qualifications (Undergraduate & Postgraduate) – must be translated to English if not originally in English. 4. Medical Registration Certificate – must be translated to English if not originally in English. 5. Copy of valid passport. 6. Copy of valid health insurance coverage (e.g., travel insurance including COVID-19 coverage). 7. Documentary proof of vaccination (with vaccination dates within the last 10 years) or serology test results for the following: ○ Varicella: documented serological evidence of immunity (VZV IgG) or 2 documented doses of vaccination. ○ Measles, Mumps, and Rubella (MMR): documented serological evidence of immunity (IgG) or 2 documented doses of vaccination. ○ Tetanus, Diphtheria, Pertussis (Tdap): vaccination record within the past 10 years (otherwise, a Boostrix is required). ○ Full COVID-19 vaccination records. ○ Chest X-ray results (for TB indicator). 8) Own laptop
